An anticoagulated patient with a bleed in the head is one of the true "drop everything" emergencies in the ICU. The individual reversal agents each have their own article; this one is about the scenario that ties them together — the anticoagulated intracranial hemorrhage — because here the reversal is only part of a fast, choreographed bundle, and the nurse is often the one making it happen in parallel rather than in sequence.
Intracranial hemorrhage isn't a fixed injury — the hematoma expands, often in the first hours, and expansion is directly tied to worse outcomes. Anticoagulation makes that expansion faster and larger. Every minute the patient stays anticoagulated is a minute the bleed can keep growing inside a closed skull. That's the entire rationale for the phrase you'll hear on the neuro service: time-to-reversal is brain. The goal is the fastest safe reversal, not the most elegant one — which is why reversal, blood-pressure control, and the CT/neurosurgery workup all happen at once.
You cannot reverse what you can't name. The single most important early action is pinning down exactly what the patient is on, because the antidote is drug-specific and reaching for the wrong one wastes irreplaceable time.

Sources for the answer: the medication reconciliation, the patient's pharmacy, family members, a pill bottle, and — critically — the timing of the last dose, which changes whether a DOAC is even still active and (for andexanet) which dose regimen applies. Get the drug name, the dose, and the time of last dose to whoever is ordering reversal.
Reversing the anticoagulant addresses one driver of hematoma expansion; elevated blood pressure is the other. In spontaneous ICH, aggressive early lowering of a markedly elevated systolic pressure is a standard part of limiting expansion, typically with a titratable IV agent (such as a nicardipine or clevidipine infusion, or labetalol) to a provider-set target — often a systolic in the 140s range, but the exact goal is individualized and protocol-driven. The nurse is usually running this infusion and titrating to target while reversal is being drawn up. The two efforts — reverse the drug, drop the pressure — proceed together.

Once the anticoagulant is neutralized, the patient's original reason for anticoagulation — atrial fibrillation, a mechanical valve, a recent clot — is now unopposed, and every reversal agent adds some thrombotic risk of its own. So the post-reversal watch includes new stroke, MI, DVT, and PE, alongside the ongoing neuro monitoring for rebleeding or expansion. The decision of if and when to restart anticoagulation after an ICH is a genuinely hard, individualized call the team makes over the following days — balancing the risk of another bleed against the risk of a clot. Your serial assessments feed that decision.
In an anticoagulated intracranial hemorrhage, the hematoma is growing and the clock is the enemy, so reversal has to be fast and drug-specific. Nail down exactly which anticoagulant the patient takes and when they last took it, match it to the right antidote (PCC + vitamin K for warfarin, idarucizumab for dabigatran, andexanet or PCC for Xa inhibitors, protamine for heparin), and give it while simultaneously controlling blood pressure and mobilizing CT and neurosurgery. Then watch both directions — for rebleeding and for the thrombosis that unopposed disease and the reversal agent can bring. The nurse who keeps these lanes moving in parallel, rather than one after another, is the one who saves the tissue.
