Acute bacterial meningitis is one of the few infections where the clock is measured in minutes and the intervention is stunningly simple: give the antibiotics. Every hour of delay raises the odds of death and permanent brain injury, and the most common reason antibiotics are late is waiting for a test. The ICU nurse's job is to make sure that wait never happens.
The classic diagnostic test is a lumbar puncture (LP) to sample cerebrospinal fluid. But if the LP will be delayed for any reason — the patient needs a CT first, the procedure is not immediately available, the patient is unstable — the antibiotics do not wait. Blood cultures are drawn, and empiric antibiotics plus steroids are given immediately. A dose of antibiotics given an hour before the LP does not meaningfully change the CSF cell counts and chemistries in that window, and cultures from the CSF may still grow or be supplemented by blood cultures and molecular testing. Nothing about the workup is worth a delay that costs neurons.
An LP in a patient with dangerously raised intracranial pressure risks precipitating herniation. So certain features prompt a CT head before the LP to look for a mass effect: new focal neurologic deficits, seizures, a significantly depressed or declining level of consciousness, immunocompromise, or papilledema. Crucially, ordering the CT is a reason to give antibiotics first — the imaging is what would otherwise create the delay the rule is designed to prevent.
In many adults with suspected bacterial meningitis, dexamethasone is given to reduce the inflammatory response that drives much of the brain injury and hearing loss (especially with pneumococcus). The timing is specific and easy to get wrong:
Once the LP is done, the CSF pattern usually distinguishes bacterial from viral meningitis. Nurses do not interpret this in isolation, but recognizing the picture speeds care:

The most memorable bacterial signature is low CSF glucose with high protein and a flood of neutrophils. A Gram stain and cultures then name the organism and let the team narrow the antibiotics.
Because the organism is unknown at first, empiric therapy is broad and age/risk-adjusted — typically a third-generation cephalosporin (like ceftriaxone) plus vancomycin, with ampicillin added at the extremes of age or immunocompromise to cover Listeria, and antivirals if HSV encephalitis is on the table. Your job is fast, correct administration and drawing levels (e.g., vancomycin) as ordered; the narrowing happens once cultures return.
Meningitis kills through several mechanisms, and each is a nursing surveillance target:
Until Neisseria meningitidis (meningococcus) is excluded, the patient goes on droplet precautions — a mask within close range, ideally for the first 24 hours of effective antibiotics. Meningococcus spreads person-to-person, and close contacts (including staff with unprotected exposure to secretions during intubation or suctioning) may need post-exposure prophylaxis. Flag your exposure early rather than after the fact.
Bacterial meningitis rewards speed and punishes hesitation. Draw the cultures, but do not let the lumbar puncture — or a CT that must precede it — delay antibiotics. Give dexamethasone with or just before the first antibiotic dose, then shift into critical-care surveillance for rising ICP, seizures, septic shock, and the meningococcal rash, all while keeping the patient (and yourself) behind droplet precautions. The nurse who gets the drug hanging fast changes the outcome more than almost anything downstream.
