Esmolol is the beta-blocker you can take back. Its effect appears in minutes and fades almost as quickly, which makes it the safest way to test a beta-blockade in a fragile patient — if the heart rate drops too far or the pressure sags, you just stop and it's gone in minutes. That controllability makes it a favorite in both the ICU and the operating room.
Esmolol selectively blocks beta-1 receptors in the heart, slowing heart rate, reducing contractility, and blunting the blood-pressure and rate response to sympathetic stimulation. What sets it apart is metabolism: it's broken down by red-blood-cell esterases, independent of the liver or kidneys, giving it a half-life of roughly nine minutes. Start it, and full effect is minutes away; stop it, and it's mostly gone within 10–20 minutes.

Because it's so brief, esmolol is often titrated aggressively to a heart-rate or blood-pressure target, with the security that any overshoot self-corrects quickly.
Rapid rate control in atrial fibrillation/flutter or sinus tachycardia, especially when you're unsure the patient will tolerate a beta-blocker. Aortic dissection, to bring heart rate and shear force (dP/dt) down fast — frequently before or alongside a vasodilator like nitroprusside, so the beta-blockade blunts reflex tachycardia. Thyroid storm and perioperative sympathetic surges (intubation, extubation, emergence) round out its niche.
Hypotension. The most common adverse effect, which is why loading doses are often skipped in unstable patients.
Bradycardia and heart block. Watch the monitor; hold or stop for excessive slowing.
Decompensated heart failure. Blunting contractility can worsen a failing pump — use cautiously.
Bronchospasm. Cardioselectivity is relative and fades at higher doses; use caution in severe reactive airway disease.
Masking hypoglycemia. Beta-blockade can blunt the adrenergic warning signs of low blood sugar in diabetics.
Esmolol is the ultra-short beta-blocker: cardioselective, titratable to the minute, and reversible simply by stopping the drip. It's the smart choice for rapid rate control, aortic dissection, and blunting sympathetic surges in patients you're not sure can tolerate beta-blockade. Respect its hypotension, bradycardia, and heart-failure cautions, and you'll wield one of anesthesia's most-used control drugs with confidence.
