When a patient in severe alcohol withdrawal is burning through escalating benzodiazepine doses and still agitated, tremulous, and tachycardic, the team starts looking for a partner drug that works by a different mechanism. Increasingly that partner is a low-dose ketamine infusion. Used as an adjunct — not a replacement — ketamine targets the exact receptor system that makes late withdrawal so benzodiazepine-resistant.
Benzodiazepines are still first-line for alcohol withdrawal because they restore GABA-mediated inhibition and prevent seizures. But in the sickest patients — those heading into delirium tremens — the driving pathology shifts toward glutamate excess at NMDA receptors. You can keep climbing the benzo dose (and many protocols do, reaching alarming cumulative amounts), but you're pushing on the GABA side of a problem that has become largely glutamatergic. That's when you see "benzodiazepine-resistant" withdrawal: huge doses, deepening sedation, and still-present autonomic storm. Adding an NMDA antagonist addresses the untreated half of the receptor imbalance.
Ketamine is an adjunct, layered on when benzodiazepines alone aren't controlling symptoms and the team wants to avoid or limit intubation. A typical severe-withdrawal escalation looks like this:

The appeal of adding ketamine before propofol is that ketamine is benzodiazepine-sparing and generally preserves respiratory drive and airway reflexes. Getting withdrawal under control without intubating a patient is a meaningful win — it avoids ventilator days, sedation cascades, and delirium.
For alcohol withdrawal, ketamine is used at low, sub-anesthetic (analgesic/adjunct) doses — far below procedural-sedation or induction doses. Institutions vary, but low continuous infusions in the range of roughly 0.1–0.5 mg/kg/hr are common, sometimes with a small loading dose, titrated to symptom control and sedation goals. This is a protocol- and pharmacist-driven decision; always run your unit's specific order set. The point to internalize as a nurse is that this is a low-dose adjunct infusion, not the dissociative dose used for intubation — if someone writes for an induction-range dose "for withdrawal," question it.
Ketamine is a sympathomimetic — it raises heart rate and blood pressure — which matters in a population already tachycardic and hypertensive from withdrawal. Watch for:

Don't forget the withdrawal fundamentals ketamine doesn't fix: aggressive thiamine before glucose to prevent Wernicke, electrolyte repletion (magnesium, potassium, phosphate), and seizure vigilance. Ketamine calms the receptor storm; it doesn't replace banana bags done right or CIWA-driven benzo dosing.
Be cautious in patients where a further catecholamine surge is dangerous — poorly controlled hypertension, active coronary ischemia, or known severe cardiac disease. Ketamine can raise intracranial and intraocular pressure, so consider the neuro picture. And remember the practical reality: many withdrawal patients have co-ingestions and unknown histories, so the team weighs ketamine's sympathetic effects against the alternative of runaway benzodiazepine escalation and intubation.
