Every fresh cardiac surgery patient bleeds a little — the question the CTICU nurse is constantly answering is whether it is the expected ooze that will settle, a medical (coagulopathy) problem you can correct at the bedside, or a surgical bleeder that needs to go back to the operating room. Getting that judgment right, and catching the moment bleeding turns into tamponade, is one of the highest-stakes skills on the unit. This is a practical walk through reading the chest tubes and recognizing when the picture has crossed a line.
Chest tube output is watched on a tight schedule — typically hourly, and more often when it is high. Three features matter. The rate tells you how fast blood is leaving; many units flag sustained output above a threshold (often cited around 100–200 mL/hr, and higher volumes over the first hour) as a trigger to escalate, but the exact number depends on patient size and your protocol. The trend is more informative than any single hour: bleeding that is decreasing hour over hour is reassuring; bleeding that is flat-high or climbing is not. The character matters too — bright red, brisk, and non-clotting suggests active surgical bleeding, while darker, slowing drainage is more consistent with settling ooze.
The team is always sorting bleeding into two buckets, because they have different fixes.

Medical bleeding is corrected by attacking its causes: reverse residual heparin with protamine if indicated, transfuse platelets and give cryoprecipitate or fibrinogen for the fibrinogen-depleted patient, give fresh frozen plasma for factor deficiency, consider antifibrinolytics per protocol, and — easy to forget — warm the patient, because a cold patient cannot clot no matter how many products you give. When bleeding is massive, your unit's massive transfusion protocol keeps ratios balanced and product flowing.
To keep tubes from clotting, nurses have traditionally "stripped" or "milked" them. This is genuinely controversial. Vigorous stripping generates very high negative intrathoracic pressure that can damage tissue and grafts, and evidence has not shown it reliably improves outcomes or prevents tamponade. Many cardiac programs now discourage aggressive stripping in favor of gentle milking or hands-off drainage, and some prohibit it entirely. Follow your unit's specific policy — this is a place where "the way I was taught" and "the current protocol" often disagree, and the protocol wins.
Cardiac tamponade after surgery is blood accumulating in the pericardial space (or a localized clot compressing one chamber) faster than the space can accommodate, squeezing the heart so it cannot fill. Because the post-op pericardium can be clotted and loculated, the classic echocardiographic signs may be atypical, and a localized clot can compress just the right atrium or right ventricle. If tamponade is developing or surgical bleeding will not stop, the patient returns to the operating room — or, if arresting or too unstable to transport, the chest may be reopened at the bedside. Every CTICU keeps a sterile emergency sternotomy (chest-reopening) tray immediately available, and the nurse's job when this is called is to get that tray, the light, and the extra hands to the bedside without delay, keep resuscitating, and support the family who is watching a very frightening event.
