Protamine is heparin's antidote — the drug that neutralizes anticoagulation when bleeding can't wait or a cardiac case comes off bypass. It works fast, but it's one of those reversal agents that can cause a crisis of its own if you push it too quickly, which is why it's given slowly and watched closely.
Protamine is a positively charged protein that binds the negatively charged heparin molecule, forming an inactive complex and neutralizing its anticoagulant effect within minutes. It's used when heparin anticoagulation must be reversed — serious bleeding, or routinely at the end of cardiopulmonary bypass to restore normal clotting.
The dose is based on how much heparin is still active, which depends on how much was given and how recently. The reference point is roughly 1 mg of protamine per 100 units of heparin remaining, but because heparin has a short half-life, you count only the heparin given in the last few hours — less protamine is needed the more time has passed since the heparin. There's also a maximum single dose, because excess protamine has a mild anticoagulant effect of its own and adds reaction risk. For continuous heparin infusions, dosing considers the amount infused over the preceding couple of hours.
Given too fast, protamine causes acute hypotension, bradycardia, flushing, and sometimes a severe pulmonary vasoconstriction/hypertension reaction. The mitigation is simple and strict: administer it slowly (over about 10 minutes for a typical dose) with the patient on continuous monitoring, ready to stop and support blood pressure. This is a drug where the rate matters as much as the dose.
Protamine can cause true anaphylaxis and severe anaphylactoid reactions. Certain patients carry higher risk: those with fish allergy (protamine is derived from fish sperm), patients who use NPH insulin or other protamine-containing insulins, men who have had a vasectomy, and anyone with prior protamine exposure (such as previous cardiac surgery). Knowing these before administration lets the team pretreat or prepare. Have epinephrine and airway support ready.

After cardiac surgery, patients can experience heparin rebound — a return of anticoagulation hours after apparently adequate reversal, as heparin redistributes from tissues back into the circulation while the protamine has cleared. This shows up as renewed bleeding or a rising ACT/aPTT post-op. It's a known phenomenon that may call for additional protamine, so post-cardiac-surgery bleeding is watched with this in mind.
Protamine is a fast, effective heparin antidote with two personalities: lifesaver when bleeding or coming off bypass, and hazard if pushed carelessly. Dose it to the heparin still active, give it slowly over about ten minutes with full monitoring, know which patients are prone to anaphylaxis, and remember its limits — partial for LMWH, useless for fondaparinux and DOACs. After cardiac surgery, keep heparin rebound on the radar. Handle it with that respect and it does exactly what critical care needs it to.
