Valproate is the third member of the status epilepticus second-line trio, alongside levetiracetam and fosphenytoin. It's a broad-spectrum anticonvulsant with a useful advantage — it's hemodynamically gentle — but it carries a distinctive set of metabolic and hematologic risks that make it a drug you monitor as much as administer.
Valproic acid works through several mechanisms — enhancing GABA, blocking sodium channels, and modulating calcium currents — giving it broad anticonvulsant activity across seizure types. In the ICU it's given IV for status epilepticus and also used for certain other indications (migraine, mood stabilization) that you may see continued in admitted patients.
After a benzodiazepine breaks (or fails to break) the seizure, a second-line agent is loaded to prevent recurrence. Trials comparing valproate, levetiracetam, and fosphenytoin found them broadly equivalent, so choice often comes down to patient factors and contraindications. Valproate's hemodynamic stability makes it attractive when hypotension is a concern — unlike fosphenytoin, it doesn't demand the same infusion-rate cardiovascular caution. A load is typically in the range of 40 mg/kg IV.

Valproate can raise ammonia and cause a hyperammonemic encephalopathy even when liver function tests are normal. A patient who becomes newly lethargic, confused, or encephalopathic on valproate deserves an ammonia level, not just a "worsening delirium" note. It's treatable — often by stopping the drug and, in some cases, giving L-carnitine — but only if it's recognized. This is a classic source of confusion because the LFTs can look fine.
Hepatotoxicity. Valproate can cause serious, occasionally fatal liver injury, with highest risk in very young children and in mitochondrial disorders. Monitor LFTs and watch for signs of hepatic failure.
Thrombocytopenia and platelet dysfunction. It commonly lowers the platelet count and can impair platelet function, raising bleeding risk — relevant before procedures and in the already-bleeding patient. Monitor the CBC.
Pancreatitis. A rare but serious idiosyncratic reaction; new abdominal pain and elevated lipase on valproate should prompt evaluation.
Teratogenicity. Valproate is one of the most teratogenic anticonvulsants and is avoided in pregnancy and in people who may become pregnant when alternatives exist.
Because so much of valproate's safety is in surveillance, the nursing role centers on trends: mental status (ammonia), liver labs, platelets and bleeding, and any abdominal complaints. Drug levels guide both efficacy and toxicity. Coordinate with pharmacy on interactions — valproate is protein-bound and interacts with other anticonvulsants, and can displace or be displaced by other highly bound drugs.
IV valproate is a solid, hemodynamically friendly second-line anticonvulsant for status epilepticus — a real advantage in the unstable patient. But it earns its keep only if you watch what it can do metabolically and hematologically: check an ammonia for any new encephalopathy even with normal LFTs, monitor liver function and platelets, keep pancreatitis in mind, and avoid it in pregnancy. Administered as a gentle load and monitored with those specifics in view, it's a valuable third option when levetiracetam or fosphenytoin aren't the right fit.
