Vasopressor Guide for ICU Nurses: Mechanisms, Dosing, and Titration

Knowing what a vasopressor does isn't enough. You need to know why it does it, what it will do to your patient's hemodynamic profile, and when to call the physician versus when to titrate and watch. This is the guide you actually need.

The Adrenergic Receptor Foundation

Every vasopressor has its effect by binding to adrenergic (or other) receptors. Know these receptors and the drugs fall into place:

Norepinephrine (Levophed) — First-Line for Septic Shock

Receptor profile: Primarily α1 + moderate β1. Strong vasoconstriction + modest positive inotropy.

Hemodynamic effect: ↑MAP via ↑SVR. HR may decrease reflexively (baroreflex). Cardiac output may decrease slightly due to increased afterload if cardiac function is borderline.

Dose range: 0.01–3 mcg/kg/min (most units start ~0.05 and titrate)

Titration goal: MAP ≥65 mmHg (or per physician order). Titrate by 0.02–0.05 mcg/kg/min every 5–15 minutes until goal or ceiling.

Nursing pearls: Norepinephrine is the vasopressor for distributive shock. It doesn't fix cardiogenic shock — if MAP doesn't respond and CO appears low (cold extremities, rising lactate despite resuscitation), think cardiogenic component and consider dobutamine addition.

Vasopressin — Second-Line Addition

Mechanism: V1 receptor agonist. Directly stimulates vascular smooth muscle contraction, independent of adrenergic system. Endogenous vasopressin is depleted in prolonged septic shock.

Dose: Fixed at 0.03–0.04 units/min. This is NOT a titrating drug. You give a fixed dose and use it to spare norepinephrine.

When to add: When NE requirement is ≥0.25 mcg/kg/min. Adding vasopressin often allows NE reduction.

Nursing pearls: Vasopressin causes coronary and mesenteric vasoconstriction at high doses. Fixed dose = less of this risk. Watch for digital ischemia with high-dose, prolonged use.

Epinephrine — High Acuity Reserve

Receptor profile: α1 + β1 + β2 at all doses. Strong vasoconstriction + strong positive chronotropy and inotropy.

Use cases: Anaphylaxis (IM or IV), refractory vasodilatory shock (when NE + vasopressin aren't enough), cardiogenic shock with low CO, post-cardiac arrest.

Dose range: 0.01–0.5 mcg/kg/min for hemodynamic support. Higher doses used in ACLS.

Nursing pearls: Lactic acidosis with epinephrine is common and can be a glycolytic effect rather than tissue hypoperfusion. Don't dose-escalate NE/vasopressin just because lactate rises after epinephrine is started — trend the clinical picture.

Dopamine — Less Used, Still Know It

Dose-dependent effects:

Current role: Less used since a major RCT showed increased arrhythmias vs. norepinephrine in septic shock. Still seen in cardiogenic shock management in some centers.

Dobutamine — For Low Cardiac Output States

Receptor profile: Primarily β1 + mild β2. Strong positive inotropy and mild vasodilation. Does NOT increase SVR.

Use cases: Cardiogenic shock, acute decompensated heart failure, sepsis-induced cardiomyopathy.

Dose range: 2–20 mcg/kg/min.

Nursing pearls: Dobutamine alone in distributive shock = disaster. It drops SVR in a patient who already has low SVR. It's used in ADDITION to vasopressors when there's a cardiogenic component. Watch for tachyarrhythmias.

Phenylephrine — Pure Alpha

Mechanism: Pure α1 agonist. Vasoconstriction only. No inotropy.

Use cases: Neurogenic shock, spinal shock, anesthesia-induced hypotension, tachyarrhythmias where β1 stimulation would worsen rate.

Dose range: 0.5–9 mcg/kg/min.

Nursing pearls: Reflex bradycardia is expected. If HR drops significantly, assess for hemodynamic impact and notify provider. Not ideal in cardiogenic shock — pure vasoconstriction increases afterload without improving CO.

When to Call vs. When to Titrate

The rule most experienced ICU nurses follow: titrate within the current order range and notify for MAP trends that don't respond. Notify immediately for: new atrial fibrillation in a patient on dopamine/epinephrine, ischemic symptoms on high-dose phenylephrine, any vasopressor at ceiling dose without MAP response, and any vasopressor in a patient who wasn't on one two hours ago.
