Acute liver failure (ALF) is the sudden loss of liver function in a person without pre-existing chronic liver disease, developing over days to weeks and defined by the appearance of encephalopathy and coagulopathy. It is uncommon but devastating, because the liver does so many jobs at once — clotting factors, ammonia clearance, glucose production, drug metabolism — that its failure hits every organ system almost simultaneously. For the ICU nurse, ALF is one of the most demanding admissions there is: the patient can look stable and then deteriorate into cerebral edema and multi-organ failure within hours, and the whole effort is a race to either regenerate the liver or reach transplant.
The liver is the body's chemical factory, and ALF is a factory-wide shutdown. It stops making clotting factors, so the INR rises and the patient becomes coagulopathic. It stops clearing ammonia, which climbs and crosses into the brain, driving hepatic encephalopathy and, at high levels, cerebral edema. It stops storing and releasing glucose, so patients slide into hypoglycemia. It loses its role in immune defense and vascular tone, so patients become vulnerable to infection and a distributive, vasodilated shock that resembles sepsis. The rising INR here is a marker of how sick the liver is — not automatically a bleed to be corrected.
Identifying the cause changes management immediately. Acetaminophen (paracetamol) toxicity is the single most important cause to catch, whether from a single large overdose or a "therapeutic misadventure" of repeated supratherapeutic dosing, because N-acetylcysteine is a specific antidote and improves outcomes even when given late and even in non-acetaminophen ALF. Other causes include idiosyncratic drug reactions, viral hepatitis, autoimmune hepatitis, ischemic "shock liver," mushroom poisoning, and pregnancy-related liver disease. A careful medication and exposure history — including over-the-counter combination products that hide acetaminophen — is a genuinely life-saving piece of nursing assessment.

The complication that most often kills in ALF is cerebral edema with intracranial hypertension, driven largely by ammonia crossing into the brain. As encephalopathy deepens toward high grades, the patient is at risk of brain herniation. Nursing care becomes neuroprotective: keep the head of the bed elevated around 30 degrees and the neck midline to favor venous drainage, cluster care and minimize noxious stimulation that can spike intracranial pressure, and treat agitation, fever, and seizures aggressively because all raise cerebral metabolic demand. A patient who is becoming difficult to rouse, developing sluggish pupils, or showing rising blood pressure with a falling heart rate is showing danger signs that warrant an immediate call — the airway may need to be secured before transport.
It feels counterintuitive, but a high INR in ALF is generally not corrected with fresh frozen plasma unless the patient is actively bleeding or needs an invasive procedure. Two reasons: the INR is the single best real-time marker of liver function and prognosis, and correcting it erases that signal the transplant team relies on; and ALF actually produces a rebalanced hemostatic state — both pro- and anticoagulant factors fall — so most patients are not spontaneously bleeding despite alarming numbers. Vitamin K is often given to exclude a reversible contributor. If bleeding does occur or a procedure is planned, blood products are used per the team's plan, and a massive transfusion protocol may be needed for major hemorrhage.
NAC is started early for suspected acetaminophen toxicity and is frequently used in non-acetaminophen ALF as well. Beyond that, care is supportive and aggressive: dextrose for glucose, careful fluid and vasopressor support for the distributive shock, low threshold for antibiotics because infection is common and can knock a patient off the transplant list, and CRRT for kidney failure or for ammonia and fluid control. Running alongside all of it is the transplant evaluation. Prognostic criteria (such as the King's College criteria) help decide who cannot survive without a new liver, and time matters enormously — early contact with a transplant center, before the patient becomes too unstable to move, is one of the highest-value actions in the whole admission.
Acute liver failure is the rapid collapse of a previously healthy liver, defined by encephalopathy and coagulopathy, that assaults the brain, clotting, glucose, circulation, and kidneys all at once. Catch the acetaminophen cause and give NAC early, protect the brain against cerebral edema with neuroprotective positioning and vigilant neuro checks, chase hypoglycemia relentlessly, resist the urge to reflexively correct the INR, cover aggressively for infection, and get the patient in front of a transplant center before they are too sick to travel. It is intense, unforgiving work — and skilled nursing is often what carries the patient across the gap to recovery or transplant.
