Hepatic encephalopathy (HE) is the reversible decline in brain function that happens when a failing liver can no longer clear the toxins — chiefly ammonia — that a healthy liver normally detoxifies. It ranges from subtle personality change and day-night reversal all the way to deep coma, and it is one of the most common reasons a patient with liver disease lands in the ICU. The encouraging part is that HE is usually reversible: treat the trigger, lower the ammonia burden, and the brain often clears. The ICU nurse sits at the center of that recovery — assessing mental status, protecting the airway, delivering lactulose, and hunting down the precipitant.
Gut bacteria produce ammonia as they break down protein. Normally the liver extracts that ammonia from portal blood and converts it to urea for excretion. In cirrhosis or acute liver failure, two things happen: the liver cells can't do the conversion, and blood shunts around the liver through collateral vessels, so ammonia bypasses detoxification and reaches the systemic circulation. It crosses into the brain, where it disturbs neurotransmission and causes astrocytes to swell — producing the confusion, and in severe cases the cerebral edema seen in acute liver failure. Ammonia is the useful mental model, but a single ammonia level correlates poorly with severity; HE is a clinical diagnosis, graded at the bedside, not by a lab number.
The West Haven criteria give a shared language for how deep the encephalopathy runs. Learning to grade it — and to notice a change of even one grade — is core nursing work, because a rising grade signals both a worsening liver and a rising airway risk.

Asterixis — the coarse flap when the patient holds the wrists extended — is a classic, quick bedside sign of grade 2 to 3 HE and worth checking on rounds.
HE rarely worsens for no reason. Something tipped the patient over, and finding it is often more important than any drug. The classic precipitants are infection (including spontaneous bacterial peritonitis), a GI bleed that dumps a protein load of blood into the gut, constipation, dehydration and over-diuresis, electrolyte disturbances (especially low potassium and metabolic alkalosis), kidney injury, and sedating medications. A nurse who notices a patient hasn't stooled in days, is running a fever, has a dropping hemoglobin, or was just given a benzodiazepine has likely found the trigger — and flagging it changes the whole course.
The workhorse treatment is lactulose, a non-absorbable sugar that does two things: it draws ammonia into the gut and traps it for excretion, and it acidifies the colon and speeds transit so less ammonia is absorbed. It is dosed to effect — titrated to roughly two to three soft stools per day, given orally, by tube, or by retention enema in a comatose patient who can't swallow safely. Too little and the HE doesn't clear; too much and the patient becomes dehydrated and hypokalemic, which itself worsens HE. Rifaximin, a poorly absorbed antibiotic that reduces ammonia-producing gut bacteria, is added to lactulose and is especially useful for preventing recurrence. Tracking stool output is not a menial task here — it is how the primary therapy is dosed.
As the grade climbs, the two dominant nursing concerns are the airway and safety. A patient sliding from grade 2 toward grade 3 to 4 is losing the ability to protect their airway and is at real risk of aspiration; escalate early rather than waiting for a coma. Meanwhile the confused, disinhibited patient is a serious fall and safety risk — reorientation, a safe environment, and close observation matter more than restraints, which can increase agitation. Serial, documented neuro checks are the backbone: a patient who is one grade worse than your last assessment is telling you the ammonia burden or the trigger is winning, and that is the moment to act.
Hepatic encephalopathy is reversible brain dysfunction from a liver that can no longer clear ammonia, graded at the bedside from subtle confusion to coma. Treatment is lowering the ammonia with lactulose titrated to two to three soft stools a day, adding rifaximin, and — just as importantly — finding and fixing the precipitant, whether that's infection, a bleed, constipation, dehydration, electrolytes, or a sedative. Avoid worsening it with benzodiazepines, protect the airway as the grade climbs, keep the confused patient safe, and treat any drop in level of consciousness as an event that needs an immediate response.
