A patient with a history of an abdominal aortic aneurysm repair — maybe years ago, maybe with a synthetic graft you have to dig for in the chart — has an episode of gastrointestinal bleeding. Some hematemesis, or blood per rectum, that stops on its own. The vitals recover. The endoscopy is unremarkable or shows no clear source. Everyone relaxes. Then hours or days later the patient exsanguinates. This is the signature and the tragedy of aortoenteric fistula: a small, self-limited "herald bleed" that is not a false alarm but a countdown, and the window to act on it is measured in hours.
The duodenum drapes directly across the front of the aorta, so anything that erodes forward from the aorta — or backward from the gut — can create a channel between the two. There are two flavors. A secondary aortoenteric fistula, the common one, arises after aortic surgery: a synthetic graft (or its suture line/anastomosis) chronically rubs and pulses against the adjacent duodenum, often with a smoldering graft infection, until it erodes through the bowel wall. This can happen months to many years after the original repair, which is exactly why it gets missed — the surgery is ancient history in the patient's mind. A primary aortoenteric fistula is rarer and occurs without prior surgery, when a large atherosclerotic or inflammatory aneurysm erodes directly into the duodenum. Either way, once a communication exists between a high-pressure artery and the gut lumen, you have a conduit for catastrophic bleeding. The herald bleed happens when a small breach seals temporarily with clot; the massive bleed happens when that clot gives way.
The presentation is deceptively mild at first, which is the whole danger. The classic sequence is a herald (sentinel) GI bleed that stops spontaneously, a symptom-free interval, then exsanguinating hemorrhage. Some patients also have abdominal or back pain and fever if graft infection is present.

The diagnostic workhorse in a stable patient is CT angiography, which can show the fistula, graft-adjacent gas or fluid suggesting infection, and active contrast extravasation. Upper endoscopy is often done to look for other bleeding sources but frequently does not reach or reveal the distal duodenal fistula — a negative scope must never be taken as reassurance in this population. If the patient is hemorrhaging and unstable, there is no time for imaging: the destination is the operating room.
Aortoenteric fistula is a surgical disease; nursing priorities are about buying time and speed. Treat it as a massive hemorrhage in waiting: establish large-bore IV access (two sites), send a type and crossmatch and activate the massive transfusion protocol readiness, and resuscitate with blood products rather than crystalloid-heavy strategies. Apply continuous monitoring, keep the patient NPO, and be prepared for airway protection if bleeding becomes torrential. Escalate immediately to vascular surgery and interventional radiology — definitive treatment is operative graft excision with revascularization or, increasingly, an endovascular stent-graft as a bridge to control hemorrhage before definitive repair, plus long-term antibiotics when infection is involved. Your assessment role is continuous: trend vitals and mental status for the swing from compensated to decompensated shock, monitor and quantify ongoing GI output, keep blood products flowing and warmed, and track coagulation and calcium during large-volume transfusion. Above all, communicate the history loudly — make sure every member of the team knows this patient has an aortic graft or aneurysm, because that single fact reframes an ordinary GI bleed as a vascular catastrophe.
Aortoenteric fistula is a connection between the aorta and the duodenum, usually from an old aortic graft eroding into the bowel, that announces itself with a small self-limited herald bleed before killing with a massive one. The history — a prior aneurysm repair or a known aneurysm — is the key that turns a routine GI bleed into a five-alarm emergency, and a non-diagnostic endoscopy provides no safety. The nurse who remembers this pattern secures large-bore access, mobilizes blood products and the massive transfusion protocol, drives emergent CT angiography or a trip to the OR, and makes sure the graft history is front and center for the whole team. In this diagnosis, recognizing the quiet warning shot is what saves the patient from the volley that follows.
