A patient becomes acutely short of breath with new infiltrates across both lungs. It looks like pneumonia or flash pulmonary edema — until the hematocrit drifts down for no obvious reason, the oxygen requirement climbs faster than expected, and a bronchoscopy returns fluid that gets progressively bloodier with each sample. The lungs are bleeding into themselves. This is diffuse alveolar hemorrhage, a syndrome where blood fills the alveoli from the inside, and it can be easy to miss because the most intuitive sign — coughing up blood — is frequently absent.
The reason DAH is easy to overlook is that the blood stays down in the alveoli rather than coming up as obvious hemoptysis. The patient looks like any other case of acute hypoxemic respiratory failure with bilateral infiltrates — a picture shared by pneumonia, heart failure, and ARDS. What sets DAH apart is the combination of that respiratory picture with a falling blood count that is not explained by bleeding somewhere visible. When the hemoglobin drops while the lungs whiten out and the patient is not obviously bleeding from the gut or a wound, the alveoli become the suspect. In the right clinical context — a known vasculitis, a transplant patient, someone on anticoagulation, or a person with kidney involvement pointing at a pulmonary-renal syndrome — that pattern should raise DAH early rather than after days of treating the wrong diagnosis.

Because the differential is wide, the workup runs in parallel: imaging, serial blood counts, coagulation studies, an autoimmune/vasculitis panel, kidney function and urinalysis to catch a pulmonary-renal syndrome, and often bronchoscopy both to confirm the diagnosis and to exclude infection. The nurse's serial observations — the trend of the hemoglobin, the trajectory of the oxygen requirement, any hematuria — are often what assembles the picture.
The tension for the nurse to understand is that these patients often need immunosuppression at the same moment their immune-mediated disease is destroying their lungs, which raises infection risk down the line — so surveillance for secondary infection is part of ongoing care. Steroid-related effects (hyperglycemia, mood changes, blood-pressure effects) are watched for as well.
Respiratory surveillance is central: continuous pulse oximetry, work of breathing, the trend of oxygen requirements, and readiness to escalate support quickly if oxygenation slips. Track the hemoglobin/hematocrit trend closely — a continued drop signals ongoing bleeding — and monitor coagulation values, platelet counts, and any signs of bleeding elsewhere. For ventilated patients, standard lung-protective care and meticulous suctioning (expect blood-tinged secretions) apply, along with attention to hemodynamics. Because immunosuppression and steroids are cornerstones, watch for hyperglycemia, signs of new infection, and steroid side effects, and enforce careful hand hygiene and line care. Administer immunosuppressants and any plasma-exchange therapy per protocol, and keep the family informed — a diagnosis that can involve autoimmune disease, the kidneys, and the lungs at once is frightening and often unfamiliar to patients.
