Clevidipine is the vasodilator you reach for when you want blood-pressure control that turns on a dime. It is an ultra-short-acting calcium-channel blocker delivered in a milky lipid emulsion, which makes it beautifully titratable and introduces a few cautions the other antihypertensive drips don't have. Here is what to know.
As a dihydropyridine, clevidipine blocks L-type calcium channels in arteriolar smooth muscle, producing selective arterial vasodilation and afterload reduction with little effect on veins or on cardiac contractility. What sets it apart is its metabolism: it is broken down by blood and tissue esterases, independent of liver or kidney function, giving it an extraordinarily short half-life. That is why its onset and offset are measured in minutes — you change the rate and see the pressure move almost immediately, then it clears just as fast when you stop.

Because clevidipine clears so quickly, a doubling strategy is standard: you increase aggressively at first to reach the target, then take smaller steps as you close in. The lipid vehicle limits how long a single vial and line can hang (strict aseptic handling and a time limit apply, since the emulsion supports microbial growth), so nurses follow the tubing-change and hang-time rules closely.
Beyond the vehicle, the drug's own effects to watch are reflex tachycardia, headache, and — as with any potent vasodilator — the potential to overshoot into hypotension, though the rapid offset makes that easy to correct.
Both are dihydropyridine drips for acute hypertension. Nicardipine has a longer duration, so it is smoother but slower to adjust and slower to wear off. Clevidipine's ultra-short half-life makes it the choice when you need rapid, precise, moment-to-moment control — but at higher drug cost and with the lipid cautions. Many units keep both, choosing based on how tightly and how quickly the pressure must be steered.
On the CRNA path, clevidipine is a favorite for intraoperative blood-pressure control precisely because of its titratability — the ability to blunt a hypertensive surge and then let it clear in minutes is exactly what anesthesia demands. Knowing the calcium-channel mechanism, the esterase metabolism that bypasses organ function, and the propofol-style lipid allergy carries directly into the OR.
Clevidipine is the ultra-titratable arterial vasodilator: one-minute half-life, organ-independent clearance, and the forgiveness that comes with rapid offset. Respect the egg/soy allergy, the lipid-metabolism contraindications, and the caloric load, follow the hang-time rules, and it gives you blood-pressure control few other drips can match.
