Digoxin is one of the oldest cardiac drugs still in use, and it earns its place in a specific niche: slowing the ventricular rate without dropping blood pressure. That's a gift in the hypotensive patient who can't tolerate a beta-blocker or calcium-channel blocker. The catch is a famously narrow therapeutic window that makes assessment and electrolytes matter as much as the dose.
Digoxin inhibits the sodium-potassium ATPase, which indirectly increases intracellular calcium and modestly boosts contractility (a positive inotropic effect). It also increases vagal tone, slowing conduction through the AV node — the mechanism behind its rate-controlling effect in atrial fibrillation. Unlike beta-blockers and calcium-channel blockers, it slows the rate without vasodilating, so it doesn't drop blood pressure.
Atrial fibrillation rate control. Digoxin shines when a patient in AF with rapid ventricular response is also hypotensive or in heart failure and can't safely take a rate-controlling agent that lowers pressure. It works better at rest than during exertion, so it's often combined with another agent for full control.
Heart failure with reduced ejection fraction. As an add-on, digoxin can improve symptoms and reduce heart-failure hospitalizations, though it's layered on top of the guideline-directed therapies that actually improve survival.
Digoxin's therapeutic range is tight, and the level isn't the whole picture — a "normal" level can still be toxic if the electrolytes are wrong. Because digoxin and potassium compete at the same pump, low potassium potentiates digoxin and makes toxicity more likely at any given level; low magnesium does the same. This is why nurses managing a digoxin patient watch the potassium and magnesium as closely as the drug level, and why aggressive diuresis (which drops potassium) is a setup for trouble.
Digoxin toxicity is common precisely because the margin is so small. Learn the pattern.

The saying is that digoxin "can cause almost any arrhythmia," which is why any new rhythm change in a patient on digoxin should prompt you to think toxicity, check a level, and check the potassium.
For life-threatening toxicity — dangerous arrhythmias, severe bradycardia unresponsive to standard measures, or significant hyperkalemia from acute overdose — the antidote is digoxin immune Fab (Digibind/DigiFab), antibody fragments that bind digoxin and neutralize it. In acute toxicity with hyperkalemia, note that the hyperkalemia is a marker of severity; the definitive treatment is the Fab, and aggressive calcium is generally avoided in this specific setting.
Several common drugs raise digoxin levels (including amiodarone and certain calcium-channel blockers), so a stable patient can drift into toxicity when a new medication is added. Because digoxin acts slowly, don't chase a rapid rate by stacking doses — that's a fast route to toxicity. And in atrial fibrillation with pre-excitation (WPW), AV-nodal slowing agents like digoxin are avoided.
Digoxin fills a real gap: rate control and a gentle inotropic boost without dropping blood pressure, ideal for the hypotensive AF or heart-failure patient. But its narrow window means the nursing job is as much about the potassium, magnesium, and renal function as the dose. Know the toxicity pattern — GI upset, visual halos, confusion, and arrhythmias — watch the electrolytes, and know that digoxin immune Fab is the rescue when toxicity turns dangerous.
