Heparin-induced thrombocytopenia is one of the great counterintuitive diagnoses in critical care. The platelet count drops — so your instinct says bleeding — but HIT is a clotting disorder. The very drug given to prevent clots has triggered an immune reaction that makes the blood dangerously prothrombotic. Recognizing HIT means overriding the reflex the low platelet count creates.
In HIT, heparin binds a protein called platelet factor 4 (PF4), and the immune system makes antibodies against the heparin–PF4 complex. Those antibodies then activate platelets on a large scale. The activated platelets clump into clots (consuming themselves, which is why the count falls) and spew out procoagulant material. So the low platelet count is not a bleeding signal — it is the footprint of platelets being used up making clots. The dangerous outcomes of HIT are new thromboses: DVT, PE, limb artery occlusion, stroke, MI, and clotted lines and filters.
HIT has a signature clock. In a patient with no recent heparin exposure, the platelet count typically falls 5 to 10 days after heparin is started. If the patient received heparin within the past roughly 100 days, pre-formed antibodies can cause a rapid-onset drop within hours of re-exposure. A fall that happens on day 1 with no prior exposure is unlikely to be HIT. Knowing when the heparin started — and when the count began to drop — is information the bedside nurse often owns before anyone else.
Because confirmatory antibody testing takes time, clinicians use the 4Ts score to estimate pretest probability and decide whether to act now. Each "T" scores 0, 1, or 2:

A low 4Ts score (0–3) makes HIT very unlikely and usually means you can keep looking elsewhere for the thrombocytopenia. An intermediate or high score triggers the HIT workup and, importantly, action before the labs resolve the question. Confirmation comes from an immunoassay (PF4 antibody, often reported as an optical density) and, when needed, a functional assay such as the serotonin release assay.
When HIT is suspected, every source of heparin comes off. This is more thorough than it sounds, and the nurse is the one who finds the hidden sources:
Low-molecular-weight heparin (enoxaparin) is not a safe substitute — it cross-reacts. Stopping heparin alone is also not enough, because the prothrombotic state persists for days.
Two "never yet" rules matter at the bedside:
Beyond the platelet trend, HIT announces itself through clotting complications and reactions: a newly swollen or painful limb, a cold pulseless extremity, chest pain or hypoxia (PE), stroke signs, a clotted dialysis circuit or arterial line, skin necrosis at heparin injection sites, and acute systemic symptoms (fever, chills, shortness of breath, hypertension) minutes after a heparin bolus. Any of these in a patient on heparin should raise HIT immediately. Documenting the platelet trend against the heparin start date, and flagging the pattern, is often what prompts the 4Ts calculation in the first place.
The ICU is full of low platelet counts — sepsis, medications, dilution, and DIC all cause them. Two contrasts anchor HIT: unlike DIC, HIT usually leaves the PT, PTT, and fibrinogen normal (the cascade isn't being consumed); and unlike most thrombocytopenias, HIT causes clots rather than bleeding. The timing (day 5–10) and the clotting picture together are what separate it from the crowd.
HIT flips the usual meaning of a low platelet count: the danger is clotting, not bleeding. Think of it when the count falls 5–10 days into heparin, score the 4Ts, and when suspicion is real, stop every source of heparin — flushes and coated lines included — and switch to a non-heparin anticoagulant, while withholding platelet transfusions and early warfarin. The bedside nurse who connects the dropping count to the heparin start date, and who resists the instinct to treat it as fragility, is the one who prevents the next clot.
