When a patient develops heparin-induced thrombocytopenia (HIT), you cannot give heparin — not even to flush a line. The direct thrombin inhibitors argatroban and bivalirudin become the anticoagulants of choice, and each has an organ-based reason for being picked.
Classic HIT shows up 5–10 days after heparin exposure (sooner with recent prior exposure) as a platelet drop of roughly 50% or more, often with new thrombosis rather than bleeding. Clinicians use the 4Ts score to gauge probability. The moment HIT is suspected, all heparin stops — IV, subcutaneous, flushes, and heparin-coated catheters — and a DTI starts. As a nurse, scrubbing every hidden heparin source is a real safety task.

The memory hook: argatroban = liver-cleared, so use it when the kidneys are bad; bivalirudin has renal handling, so it's often favored when the liver is bad. Both have short half-lives, which is helpful because neither has a reversal agent — stopping the drip is the "reversal," and effect fades quickly.
Both are typically titrated to a target aPTT (often around 1.5–3 times baseline, per protocol) with no bolus in HIT for argatroban in most stable patients. Check the aPTT at the protocol interval after every rate change, and don't chase a single value — trend it. Because there's no antidote, over-anticoagulation is managed by holding or reducing the infusion and letting the short half-life work.
Eliminate every heparin source at initiation. Draw aPTT (or the assay your protocol specifies) on schedule and after each titration. Watch for bleeding — these are potent anticoagulants with no reversal agent, so monitor puncture sites, drains, neuro status, and hemoglobin. Track the platelet count for recovery, which helps confirm the diagnosis and guides the warfarin transition (warfarin isn't started until platelets recover). And during any argatroban-to-warfarin overlap, flag the INR interpretation issue to the team.
