Norepinephrine is the first drug your patient in shock is likely to be on, and the one you will titrate more than any other pressor. Get comfortable with it and you understand the backbone of hemodynamic support. Here is the practical, protocol-aware picture every ICU nurse and CRNA student should carry.
Norepinephrine is an endogenous catecholamine. Pharmacologically it is a strong alpha-1 agonist with meaningful beta-1 activity and minimal beta-2 effect. The alpha-1 stimulation constricts arterial and venous smooth muscle, raising systemic vascular resistance and venous return. The beta-1 activity modestly increases contractility and heart rate, which usually offsets the reflex bradycardia you might otherwise expect from a rising blood pressure. The net effect is a reliable rise in MAP with generally preserved cardiac output, which is exactly why the Surviving Sepsis Campaign names it the first-line vasopressor in septic shock.
Norepinephrine is dosed either in mcg/min or, increasingly, weight-based in mcg/kg/min. Always follow your unit's concentration and pump library. Common ranges:

The MAP 65 target comes from the SEPSISPAM trial and Surviving Sepsis guidance: for most patients, a higher target does not improve outcomes and adds arrhythmia risk, though chronic hypertensives may benefit from a slightly higher goal per the provider. When you are climbing the dose and not getting there, that is the trigger to think about the next steps: add vasopressin as a second agent, re-check volume status, and consider whether the patient needs an inotrope or steroids.
Arrhythmias. The beta-1 activity can provoke tachyarrhythmias, especially at higher doses. Watch the monitor.
Reflex effects and end-organ perfusion. Excessive vasoconstriction can compromise perfusion to fingers, toes, and gut at very high doses. Assess distal pulses, capillary refill, and urine output as your real-world perfusion readout, not just the number on the arterial line.
Arterial line. Patients on any titrated pressor should have continuous, accurate blood-pressure monitoring — an arterial line is standard. Cuff pressures lag and mislead during rapid titration.
Never abruptly stop. Wean it down as the patient improves; an abrupt discontinuation can drop the pressure precipitously.

On the CRNA path, norepinephrine is a daily intraoperative tool for managing anesthesia-induced hypotension and vasodilatory shock. The receptor pharmacology you learn titrating it at the ICU bedside — alpha-driven pressure with a beta lift, the MAP goal, the extravasation rules — is the identical knowledge you apply pushing and infusing it in the OR. Mastering pressors as a critical-care nurse is one of the highest-yield things you can do to prepare for anesthesia school.
Norepinephrine is the workhorse vasopressor: first-line for septic shock, titrated to a MAP of 65, ideally central, and dangerous on extravasation. Watch the trend, protect the tissue, keep phentolamine in mind, and never yank it off abruptly. Build those instincts now and they carry straight into anesthesia practice.
