Phenytoin is one of the oldest second-line agents for status epilepticus, and almost every risk it carries is tied to how you give it — the rate, the line, the fluid, and the level. Fosphenytoin, its water-soluble prodrug, exists largely to make that administration safer. Here is what a nurse hanging either one needs to have cold.
The treatment ladder is benzodiazepine first, then a second-line agent, then anesthesia. Phenytoin/fosphenytoin has long been a standard second-line choice alongside levetiracetam and valproate; large trials found the three roughly comparable, so the choice often comes down to interactions, hemodynamics, and what the patient is already on. IV phenytoin is dissolved in propylene glycol at a highly alkaline pH. That vehicle is what drives the two signature hazards: cardiovascular depression when infused quickly, and severe tissue injury if it extravasates. Fosphenytoin is a water-soluble prodrug that the body rapidly converts to phenytoin, so it can be given faster, hurts far less at the site, and can even be given IM in a pinch. It is dosed in phenytoin equivalents (PE) precisely so the numbers match — 1 mg PE of fosphenytoin yields 1 mg of phenytoin.

Purple glove syndrome is progressive limb discoloration, edema, and pain that can appear during or after IV phenytoin, even without obvious extravasation, and can progress to serious soft-tissue and vascular injury. Use a large, well-placed IV, avoid small hand veins when possible, flush with saline before and after, and inspect the site frequently. Any dusky discoloration or disproportionate pain means stop and escalate. This risk is one of the strongest arguments for fosphenytoin when IV access is marginal.
Phenytoin is highly protein-bound, so the total level can mislead you in critically ill patients with low albumin: the total looks "low" while the pharmacologically active free fraction is actually adequate or high. Correct the total level for albumin (the Sheiner-Tozer equation) or, better, measure a free phenytoin level when available. Typical targets are roughly a total of 10–20 mcg/mL or a free level of 1–2 mcg/mL.
Phenytoin also follows saturable (zero-order) kinetics near therapeutic concentrations, so a small dose increase can produce a disproportionate jump in level and tip a patient into toxicity — nystagmus, ataxia, slurred speech, and lethargy. That non-linear behavior is why levels are followed carefully and why dose changes are made cautiously.
Phenytoin precipitates in dextrose-containing fluids, so it runs in normal saline only, ideally through a dedicated line or with a saline flush bracketing the dose. It is also a potent hepatic enzyme inducer with a long list of interactions — it can lower levels of many co-administered drugs, and its own level is affected by others. On a busy ICU medication list this is a frequent source of hidden problems, so it is worth flagging to pharmacy.
On the CRNA path, phenytoin is a case study in how the vehicle and kinetics — not just the molecule — define a drug's danger. The 50 mg/min limit, the saline-only rule, purple glove syndrome, and albumin-corrected levels are classic exam and safety points that also translate directly to the anesthesia and neuro-critical-care settings you will rotate through.
Phenytoin and fosphenytoin stop seizures the benzodiazepines could not, but they punish sloppy administration. Load around 20 mg/kg (or PE/kg), hold phenytoin to 50 mg/min on the monitor, run it in saline through a good line, watch the limb for purple glove, and read the level against the albumin. When access is marginal or speed matters, fosphenytoin buys you a wider safety margin.
