The nitroglycerin drip is your fast, titratable tool for chest pain, hypertensive flash pulmonary edema, and acute heart failure. It works in seconds and wears off in minutes, which makes it powerful and forgiving — as long as you respect the blood pressure and two hard contraindications. Here is the practical picture.
Nitroglycerin is converted to nitric oxide, which relaxes vascular smooth muscle. The venous system is most sensitive, so low-dose NTG primarily reduces preload — less venous return means a less congested, less strained heart, which is why it relieves pulmonary edema and angina so effectively. At higher doses it also dilates arteries (reducing afterload) and the coronary arteries (improving myocardial oxygen supply). The net effect is reduced cardiac workload and improved coronary perfusion.

NTG's onset is near-instant and its effect fades within minutes of a rate change, which makes it exceptionally titratable — perfect for the crashing hypertensive patient in flash pulmonary edema, where aggressive NTG can turn the picture around quickly. It requires special non-absorbing (non-PVC) tubing in some formulations, and continuous blood-pressure monitoring, ideally an arterial line at higher doses.
Hypotension. The main dose-limiting effect. Because NTG drops preload, hypovolemic patients are especially sensitive — the pressure can fall fast. Have fluids ready and titrate carefully.
Headache. Extremely common and sometimes severe, from cerebral vasodilation. It is a nuisance, not usually dangerous, but patients notice it immediately.
Reflex tachycardia. Can increase myocardial oxygen demand — a consideration in ACS.
Tachyphylaxis. Continuous infusion leads to tolerance within roughly 24 hours; the dose may need escalation, and nitrate-free intervals are sometimes used for chronic therapy.
On the CRNA path, nitroglycerin is a perioperative tool for controlled hypotension, myocardial ischemia, and acute hypertension. Understanding its preload-reducing mechanism, its rapid titratability, and the PDE5 and RV-infarct cautions at the ICU bedside is the same knowledge you will apply managing hemodynamics in the OR.
