Phenylephrine is the simplest pressor to understand and one of the most useful to have in your pocket — literally, as a push-dose syringe. It does one thing: squeeze the vessels. No inotropy, no rate boost. That purity makes it the right tool in very specific situations and the wrong tool in others, and knowing the difference is bread-and-butter anesthesia thinking.
Phenylephrine stimulates only alpha-1 receptors, causing arterial and venous vasoconstriction. That raises systemic vascular resistance and blood pressure. Because it doesn't touch beta receptors, it adds nothing to heart rate or contractility directly. In fact, as the pressure climbs, the baroreceptor reflex often slows the heart — the classic reflex bradycardia that defines this drug.

Phenylephrine is the classic push-dose pressor: a small IV bolus to bridge a patient through brief, sharp hypotension — after intubation meds, during a spinal, or a transient drop — while you address the cause or spin up an infusion. Every ICU and anesthesia nurse should know their institution's push-dose setup and never mix it from memory.
It shines when you want to raise pressure without adding heart rate: a tachycardic patient who's hypotensive, aortic stenosis or hypertrophic cardiomyopathy where you want tone but not tachycardia, or hypotension from vasodilation (e.g., after induction or neuraxial anesthesia). It's clean, predictable, and easy to titrate.
Reflex bradycardia. Expected, not always a problem — but in a patient dependent on heart rate for cardiac output it can backfire.
Reduced cardiac output. Increased afterload can lower output in a compromised ventricle.
Extravasation. Like other vasoconstrictors, it can injure tissue if it infiltrates; central access is preferred for prolonged infusions, though its peripheral and push-dose use is well established.
Phenylephrine is the pure squeeze: alpha-only vasoconstriction, reflex bradycardia, and no inotropy. It's the go-to push-dose pressor and the smart choice when you need pressure without tachycardia — but the wrong choice for a failing pump. Master when to grab it versus norepinephrine at the bedside, and you'll make that decision instinctively the first time a blood pressure drops in the OR.
